Acknowledgment of Risks
I understand that there are inherent risks associated with school activities, both within the classroom and during outdoor activities.
Release of Liability
I hereby release and hold harmless Beautiful Minds Learning, its staff members, teachers, and property owners from any liability for injuries or accidents that may occur.
In the event of a dispute related to any injuries or accidents, both parties agree to seek mediation before pursuing any legal action.
Assumption of Risk
I assume all risks of injury or harm to my child arising from participation in these activities.
Medical Treatment
In case of an emergency, I authorize Beautiful Minds Learning staff to seek medical treatment for my child.
